Case Management - How Can a Case Manager Help you?

Case Managers are advocates, supporters, listeners, referral sources and information providers.

The Case Managers at The Columbus AIDS Task Force are here to help. Using CATF resources and working closely with other community agencies and programs, CATF Case Mangers can assist clients with the many challenges they face.

When working with a CATF Case Manager a client can expect confidentiality, respect, and accurate information. Case Managers work with clients as partners to ensure the best quality of life possible.

It is not the policy of CATF to solicit individuals for the purpose of enrolling them in services. We do perform various outreach measures to make people aware of our services and we encourage those people who are HIV+ to contact us. If someone would like to become a client of CATF, he or she should call the agency directly. If necessary, a person may authorize someone else to call on his/her behalf for the initial inquiry; however, the enrollment in services must be done directly with the person living with HIV/AIDS.

If someone is too ill to call on their own behalf, they may request that someone else call for them. However, services are only available to individuals who desire services for themselves.

If you or someone you know is interested in Case Management at Columbus AIDS Task Force please call 614-299-AIDS (2437) and ask for a Case Manager, we will be happy to assist you.
